Floating Support is a Medway Council commissioned service to help people in their own homes or temporary accommodation. Our goal is to help people live independently and well, and we understand there is more to this than having somewhere to live. Floating Support is available to:
- Help manage tenancies.
- Help obtain the support people need in other areas of their lives.
- Reduce the risk of homelessness.
- Reduce social isolation.
- Increase confidence.
We are here for anyone going through a difficult time
How Floating Support works
We understand every person and their needs are different. Once a referral is with AMAT one of our Floating Support Workers will arrange a visit; this is where goals are discussed and a full support plan will be made. We keep in touch with clients in ways that suit them, checking progress and celebrating achievements.
By working with different organisations, services, and charities we make sure clients get the help they need, and once all goals are completed we will ask the client if they are happy to close the case.
If someone needs help in the future they can refer to Floating Support again.
To make a referral – Referrals can be made by professionals or people can refer themselves.
